Under the direct supervision of the Physical Therapist/Director of Nursing (DON), the Physical Therapy Assistant will work closely with residents and patients and will provide physical therapy assistance to meet the goals associated with the plan of care for our residents/patients who require physical functioning in order to gain independence in ambulation and other activities. There is latitude for independent decision making. Must be able to fluently communicate in or to take or give direction or guidance in the English language.


The Physical Therapy Assistant must have a thorough knowledge of the practices and procedures used in skilled nursing facility and possess strong written and oral communication skills and be able to understand written and oral instructions. Effective in establishing and maintaining rapport with staff, residents, outside vendors, and clients. Must possess a genuine interest in and concern for geriatric, chronically ill, and/or disabled persons.


  1. Regular attendance is considered an essential function of this position and is necessary for the efficient operation of the business.
  2. Provides treatment as ordered by the Physical Therapist and attending physician.
  3. Assists the Physical Therapist and Physician in evaluating the patient’s level of function. Evaluates and establishes goals based on the patient’s current level of functioning and potential improvement. Assists in developing and updating the plan of treatment.
  4. Assists in guiding and instructing therapeutic, strengthening and functional activities that are directed toward improving independence in ambulation, strength and balance.
  5. Observes, records, and reports to the physical therapist the patient’s reaction to the therapy program or any changes in the patient’s condition.
  6. Assists Physical Therapist in advising, consulting, and instructing patients in the use of prosthetic, orthotic and assistive devices. Instructs, teaches, and supervises other health care team personnel in the case of patients regarding physical therapy.
  7. Adheres to all applicable federal, state, local and company maintained standards of care and ethics and policies concerned with the practice of physical therapy.
  8. Participates in staff development activities. Participates in in-service education.
  9. Demonstrates evidence of continuing education conducted for in-service sessions.
  10. Meets productivity and utilization expectations as established by Glencroft.
  11. Assists in completing timely clinical documentation to include functional goal setting, progress and therapeutic approach.
  12. Assists in completing administrative/billing documentation for payment processing in a timely and accurate manner.
  13. Performs other duties as assigned.
