The Dementia Specialist fulfills the responsibilities of overseeing and completing Geriatric Care Assessments and implementing service plans for individuals with dementia as part of the comprehensive agency Alzheimer’s Disease Initiative.  Responsibilities also include spearheading the increasing dementia-capabilities of the agency, managing work flow of the music & memory assessments, tracking outcomes in JFS’ ETOi client data bases, collaborating with JFS’ Community Impacts team, and providing reports and data to the Director of Care Management.  


  • Assist in the development, implementation, and evaluation of the JFS Alzheimer’s Disease Initiative, a multi-faceted dementia pilot.
  • Conducts in-home Geriatric Care Assessments, entering info into ETOi data base.
  • All new clients will be contacted to schedule a home visit for an assessment, within three working days of the staff member having received the referral.
  • Track the use of a dementia screening and referral protocol across JFS programs.
  • Care Plan developed for client within three working days of completing the assessment.
  • Link clients to dementia-specific services.
  • Provides timely reporting and sharing of information with the Director of Care Management:
  • Responsible for entering touch points in ETOi accurately 98% of the time, with record-keeping to be completed in the appropriate time frame 90% of the time. All client touch points to be entered into ETOi by the 1st working day of the subsequent month 100% of the time.
  • Time sheets submitted online on time 98% of the time and mileage forms completed and submitted to supervisor according to schedule 95% of the time.
  • Reviews clients with supervisor on a quarterly basis with clients in crisis to be discussed with the Director of Care Management within one working day. 
  • Completes reports/program monitoring in the absence of the Director of Care Management.


  • Master’s degree in Social Work or behavioral science and experience working with clients who have Alzheimer’s disease and related dementia (ADRD). 
  • Minimum of five years’ experience working with frail elderly, including dementias, and mental health challenges.
  • Preferred familiarity of Jewish history, traditions and culture. 
  • PCTI (Person Center Trauma Informed) trained.


Job Specific:


  • Bi-lingual in Spanish preferred with the ability to speak, read and write Spanish fluently, including accompanying clients to medical appointment able to translate and advocate.
  • Good to excellent spelling, grammar and written communication skills 
  • Excellent telephone and oral communication skills
  • Ability to read and interpret documents such as memos, general clerical documents, project and grant reports, safety rules, instruction booklets and manuals
  • Ability to write well (e.g., memos, reports, e-mails, agendas, minutes)


  • Ability to add, subtract, multiply, and divide all units of measure using whole numbers, common fractions and decimals
  • Ability to analyze data


  • Ability to identify data reporting needs/requirements.
  • Ability to apply common sense and critical thinking to carry out instructions and make decisions within scope of authority
  • Ability to deal with problems involving several variables


  • Ability to work efficiently on line systems including JFS’ ETOi, with the ability to prepare and export reports.
  • Intermediate proficiency in Word, Excel and Outlook
  • Ability to write clear concise e-mails
  • Ability to take usable photos and design books of events using the photos and graphic art


  • Valid driver’s license; dependable accessible transportation, current auto insurance and clean DMV record.
